A Solid Start for Every Child: The Netherlands Integrates Medical and Social Care, 2009 - 2022

Abstract

Despite having a sophisticated health-care system and spending more on health care than do most countries in the world, by the early 2010s the Netherlands experienced some of the poorest perinatal-health outcomes in the European Union. Birth-related complications among women and infants were driven primarily by economic and social inequality. For example, women living in the country’s low-income neighborhoods were up to four times more likely to die during childbirth than the Dutch average. In partnership with university researchers, the municipalities of Rotterdam, Groningen, and Tilburg began tackling the problem. After discovering that the growing disparities in perinatal health outcomes were driven in large part by social and economic challenges rather than by purely medical factors, the cities set out to build integrated, multisectoral teams­—local coalitions—that brought together service providers working in both the health-care and social domains. To tailor care to an individual patient’s own circumstances, the coalitions transcended the traditional boundaries that separated physicians, midwives, municipal officials, social workers, and other service providers. They worked to integrate their records and come to agreement on ways to monitor progress, and they designed referral systems and procedural road maps to deal with specific and individual client problems. In 2018, the national Ministry of Health, Welfare and Sport expanded the use of such local coalitions to reduce early-childhood health disparities in municipalities throughout the country. By early 2022, 275 of the Netherlands’ 345 municipalities were participating in the program, dubbed Solid Start, and the new national government pledged to expand the program to every municipality in the country.

Seizing Opportunities, Strengthening Synergies: Lima Frames a Collective Strategy to Advance Early Childhood Development, 2019–2021

Abstract

Jorge Muñoz had long championed efforts to improve the lives of children in his relatively well-off district of Peru’s capital city, Lima. In 2019, he had a chance to take some of his ideas to scale. As newly elected mayor of metropolitan Lima, a city of almost 11 million, he oversaw basic services for about a third of the country’s population. At the time, a fifth of Peru’s population lived in poverty, and one in three people lived in informal settlements, where supporting families to give infants and toddlers a healthy start on life presented many challenges. The mayor directed the metropolitan government’s Social Development Department and a small interdisciplinary team of architects and social scientists (1) to identify lessons learned from pilot projects, (2) to establish new ways of assisting infants and young children, and (3) to coordinate to get the job done. When the COVID-19 pandemic broke out in the capital city in 2020, the metropolitan government and its team continued this work, using some of their newly created systems to respond to the larger challenge of caring for vulnerable populations during months of emergency lockdown measures. The national government labeled Lima’s program, which engaged residents in project development, as a promising model for helping local governments implement a countrywide strategy for the promotion of early childhood development.

Bridging the Divide: Coalition Building for Early Childhood Development in Istanbul, 2016 – 2020

Abstract

By the mid-2010s, Istanbul, the biggest city in Turkey, had developed a reputation as a bustling concrete jungle notoriously unfriendly to the 1.2 million children aged four years and younger who lived there. As part of a decade-long construction boom, multistory skyscrapers increasingly replaced green spaces and parks throughout the city. But such insufficient consideration for the developmental needs of young children was not confined to the design of public and urban spaces: in many Istanbul homes, parents worked hard to put food on the table and had little time to consider how to give their young children the best possible start in life. In February 2016, a coalition of policy research organizations and private enterprises launched an ambitious effort to persuade officials in Istanbul’s 39 districts to begin taking the needs of young children seriously. The group drew on help from a network of prominent Turkish universities and partnered with four district municipalities that agreed to join a program called Istanbul95, supported by the Bernard van Leer Foundation, a Dutch foundation. The group created a digital-mapping tool to help locate vulnerable children, conducted regular home visits to support hundreds of families, and designed new prototypes for child-friendly public spaces. This effort to embed principles of early childhood development into the work of Turkish local governments passed a milestone when, in 2019, the major metropolitan area governments of Istanbul and İzmir also agreed to join, a key step toward reaching many more children.

Municipal Turnaround in Cape Town, South Africa, 2006-2009

Abstract
In March 2006, the Democratic Alliance won elections in the city of Cape Town, taking over administrative and political control of the municipality following four years of rule by the African National Congress, South Africa’s dominant party. Helen Zille, Cape Town’s new mayor, stepped into a difficult situation. Crumbling infrastructure had eroded service delivery for years, undermining public confidence in the city government and jeopardizing the long-term economic prospects of the Cape Town metropolitan area.   Lacking the revenue and administrative capacity to address Cape Town’s infrastructure crisis, and facing a politically charged racial climate, Zille and her Democratic Alliance government initiated a package of innovative and far-reaching reforms. This case study recounts these efforts from 2006 to 2009, and describes how tough decisions to raise local revenue interacted with a program to stabilize an underskilled and demoralized city bureaucracy, reversing Cape Town’s precipitous decline.

Abstract

Emile Short recounts his experiences establishing the Commission for Human Rights and Administrative Justice in Ghana from 1993 to 2004. He goes into detail about the strategies put into place to establish the commission while avoiding any politicization and patronage within this process. He recalls how he and others set up the commission with a triple mandate encapsulating human rights, ombudsman and anti-corruption activities, and how cases were selected and prioritized. The need for effective recruitment was essential in the initial stages, as well as the training that staff would receive initially, and continue to receive.  He details the character of the individuals sought for the commission and the particular skills set that was required. Throughout, he explains the challenges that were encountered and how these were overcome.

Profile

Emile Short served as commissioner of Ghana's Commission on Human Rights and Administrative Justice from 1993 to 2004. Before that, he was head of a Ghanaian law firm for 20 years. He also served as a justice on the United Nations International Criminal Tribunal for Rwanda. He earned an LLM degree from the London School of Economics and Political Science in 1967. He also received an honorary doctorate from Northwestern University in Illinois.

Abstract
Jayanta Madhab goes into detail about the reform programs undertaken in Assam during his time as advisor to the chief minister of Assam from 2003-2009. He details the need to introduce new legislation into law in order to improve the financial situation in the state and improve funding, done through the value added tax system, and the challenges faced due to the recession and deficit financing. He talks about the need to implement reforms that will have far reaching affects across all ethnicities, minorities, and vulnerable groups, and how managing these factions and autonomous groups was essential to the reforms success. He describes the main problem in the reform process was changing the attitudes and building support within government for change through coalition building and management of insurgent groups in the region. Finally, he specifically address the need to improve employment and education opportunities as well as technical skills (such as improving agricultural practices in poor rural areas) as a means of keeping poor disaffected youth out of insurgent groups.

Jayanta Madhab is an economist who served as an advisor to the chief minister of Assam, India, from 2003 to early 2009. During his time in government he advised the chief minister on economic and financial affairs before taking up the mandate of employment generation in Assam in the chief minister’s second term. He has worked for the Asian Development Bank, as well as the North Eastern Development Finance Corporation (NEFDI). He holds a Ph.D. from the London School of Economics.

Abstract
In this interview, Alfred Brownell details his extensive involvement with the reform of management of natural resources in Liberia. Specifically, Brownell elaborates on the experiences that led him to work for the creation of the Association of Environmental Lawyers of Liberia (Green Advocates), an organization that has played a pivotal role in Brownell’s campaign for improving the country’s forestry sector. Brownell describes the steps he took in obtaining much-needed cooperation from the government and private institutions to pass legislation that would safeguard the exploitation of forest cover, empower local communities living near such forests and stem resource-related human rights violations. Brownell also provides insight into the efforts that culminated in the incorporation of the Liberian Extractive Industries Transparency Initiative (LEITI). In explaining the successes achieved, Brownell describes how coalitions were built and campaigning was organized to receive support from both the populace and international organizations. These efforts resulted in the imposition of sanctions by the United Nations Security Council. These sanctions were a crucial leveraging tool in obtaining government cooperation. Recounting the many challenges involved in the process of obtaining successful reform, Brownell stresses the importance of oversight and institutional capacity building in ensuring the sustainability of reform.

 At the time of this interview, Alfred Brownell was the president of the Association of Environmental Lawyers of Liberia (Green Advocates.) In 1997, while a law student at the University of Liberia, Brownell launched Green Advocates and put into place Liberia’s first framework environmental law. Brownell was also the lead campaigner for Liberia’s Extractive Industries Transparency Initiative, successfully pressing for the passage of legislative reform in the national forestry sector. Between 2000 and 2003, Brownell’s work helped bring about the imposition of sanctions on Liberia’s timber exports, a travel ban and an asset freeze on corrupt government officials. Through the years, Brownell has devoted himself to campaigning for national policies that stem corruption and abuses related to natural resource extraction.
